Full-time role. Pay is $18.00 to $21.00 an hour based on experience. Benefits include health insurance, dental insurance, vision insurance, sick time, PTO, 401K, EAP, life insurance (optional), and short-term/long-term disability.

About The Position

As a Community Inclusion Specialist, you’ll help individuals in Adult Care Homes (ACH) transition to Permanent Supportive Housing through the Transitioning to Community Living (TCL) program. You’ll connect members to services and supports that improve their quality of life, everything from employment and benefits counseling to medical services and educational opportunities. You’ll be the bridge between individuals and their communities, ensuring they feel empowered, included, and supported. In this role, you’ll also foster a positive work culture by staying solution-focused and demonstrating a positive attitude in all interactions, emphasizing recovery-oriented and trauma-informed traits such as patience, tolerance, positive support, and mutual respect.

 

What You’ll Do

Program Management

  • Guide individuals through their transition into community living and connect them with essential services.
  • Implement tools like Informed Decision Making and the Mattering Framework to ensure personalized support.
  • Collaborate with the Program Manager and community partners to create seamless transitions and enhance quality of life for each individual.
  • Provide accurate and necessary information to the Program Manager or Director for reporting, and work with the Accounting Manager to ensure proper billing and invoicing for services rendered through Alliance.
  • Participate in case review meetings with Alliance’s TCLI staff, as assigned, to support individuals as needed.
  • Support special projects as assigned, including tasks like coordinating project-specific information, creating documents, developing presentation slide decks, and facilitating team workgroups.

Community Engagement and Outreach

  • Partner with local organizations, churches, and nonprofits to help individuals fully integrate into their community.
  • Identify creative ways for individuals to furnish their homes and engage in local activities.
  • Work closely with Alliance Health and other stakeholders to create lasting community connections.

Member Engagement

  • Support members in accessing housing, employment, and community services.
  • Use Supportive Housing Best Practices to ensure individuals live in safe, affordable, and integrated housing.
  • Help members connect with peers and support services, empowering them to thrive in their new communities.

 

Qualifications

  • Personal history of recovery and lived experience with crisis-based support.
  • Knowledge of trauma-informed care and experience working in community support.
  • Valid Driver’s License and High School Diploma (Associate or Bachelor’s preferred in Human Services or related field).
  • Ability to build strong, empathetic relationships while maintaining confidentiality and professionalism.
  • Excellent communication skills and the ability to work independently or within a team.
  • Self-motivated and skilled in managing time and completing tasks with limited supervision.

You’ll be required to complete the following contractual training within 90 days of hire:

  • Informed Decision-Making Tool
  • Peer Support Services Policy (3 hours)
  • Documentation Training (1 hour)
  • NC Certified Peer Support Specialist (or willingness to achieve certification within 6 months).
